School Health Services 
The health and well-being of our students and staff is a top priority. A Certified Registered Nurse is on campus daily from 7:30 a.m. to 3:30 p.m., providing care, guidance, and support to students and staff. 
The Nursing Office, located in the Elementary Division campus, ensures compliance with the Puerto Rico Departments of Health and Education to maintain a safe and healthy school environment. 
Health Forms and Requirements 
All required health forms must be uploaded through our secure online system, Magnus Health. 
The Puerto Rico Department of Health and Education requires the following forms for all students: 
  • Annual Physical Exam Form – Completed and signed by a physician each year. 
  • Dental Examination Form – Required for grades K, 2, 4, 6, 8, and 10. 
  • PVAC-3 (Green Immunization Form) – Must be current and updated as booster shots are administered. 
Vaccines are generally required at ages 4, 11, and 16. For detailed guidance, refer to the CDC Immunization Schedule
Complete submission of all Magnus Health documents is required for enrollment.

Saint John’s School

Saint John’s School is a non-profit, college preparatory, nonsectarian, coeducational day school founded in 1915. The school, located in a residential area of Condado, in San Juan, Puerto Rico, has an enrollment of over 800 students from Toddler to grade twelve. Instruction is mostly in English with the exception of language courses.
